How to Properly Use a Vial Adapter and Syringe for Injectable Medications
Injectable medications can feel intimidating at first, but with the right technique and preparation, the process becomes simple and safe. This guide will walk you through how to properly use a vial adapter with either a standard syringe or an insulin syringe.
While this process is commonly used for medications like testosterone cypionate (both commercial and compounded), these same steps may apply to other injectable therapies prepared by our pharmacy.
Gather Your Supplies
1. Before getting started, make sure you have all the necessary supplies on hand. These are typically included with your prescription.
You will need:
Your medication vial
A sterile vial adapter
A syringe (or insulin syringe, if prescribed)
The appropriate injection needle (if required)
Alcohol swabs for cleaning
Before use, always check that all packaging is sealed and sterile. If anything appears damaged, opened, or missing, contact your pharmacy before proceeding.
2. Begin by removing the protective seal from your medication vial.
Compounded medications, such as testosterone, often come with a circular aluminum seal (commonly pink or blue) that must be peeled off. Other medications may have a plastic cap that snaps off easily.
Once the top is removed, clean the rubber stopper thoroughly using an alcohol swab. Allow it to air dry before moving to the next step.
3. Carefully open the sterile vial adapter package, making sure not to touch the tip.
Hold the adapter with the spike facing downward. Place your medication vial on a flat, stable surface, then center the adapter over the rubber stopper.
Press straight down firmly until the adapter clicks into place. You may feel some resistance—this is normal.
Avoid twisting or rocking the adapter during insertion, as this can compromise the seal. Once attached, the vial adapter should remain in place for the entire duration of the vial’s use.

Unlike traditional methods that often require a larger 18-gauge needle for withdrawal, the vial adapter allows you to use your injection needle from start to finish, making the process cleaner, safer, and more convenient.
Draw up the Medication
4. Before attaching your syringe, pull back on the plunger to draw in air equal to your prescribed dose.
Next, connect the syringe to the vial adapter by twisting it clockwise until it feels secure.
Inject the air into the vial. This step helps equalize pressure inside the vial, making it easier to withdraw the medication and preventing a vacuum from forming.
With the syringe still attached, turn the vial upside down so the medication flows toward the adapter. Slowly pull back on the plunger to draw up the exact prescribed dose.
Check for air bubbles. If present, gently tap the syringe to bring bubbles to the top, then adjust the plunger as needed.
Once you have the correct dose:
Carefully disconnect the syringe by twisting it counterclockwise
Leave the vial adapter attached to the vial
Attach your injection needle securely to the syringe
Your medication is now ready for administration, as directed by your healthcare provider.
For lower doses, commonly prescribed for women, your provider may recommend using an insulin syringe, which allows for more precise, small-volume dosing. When using an insulin syringe, a vial adapter is not needed. Instead, insert the syringe directly through the rubber stopper of the vial. Pull back the plunger to draw in air equal to your prescribed dose, inject the air into the vial, then invert the vial and withdraw the exact amount of medication. After removing any air bubbles, the syringe is ready for injection as directed.
